Our care team works in collaboration to assist patients once they've been discharged from an acute setting to rehabilitate and recover from their injury or illness.  We also provide long term home care for patients with behavioral health, developmental disability, and chronic health needs.  We have 5 convenient locations and serve over 1400 clients throughout the state.
