Utopia Home Care looks forward to attending the 29th Annual Connecticut Assisted Living Association (CALA) Conference and Trade Show at the Aqua Turf Club on March 21st.
Utopia partners with 8 Managed Residential Communities throughout Connecticut, providing in-house Assisted Living services for the residents who reside there. These services include nursing for wellness checks and medication management as well as home health aide assistance with Activities of Daily Living (ADLs).

Utopia Home Care is a Medicare certified, state DPH licensed, and CHAP accredited home care agency which provides registered nurse case management, physical therapy, occupational therapy, and home health aide services for clients in need of both long-term and short-term post acute care. We have 5 branch offices in Connecticut including Bridgeport, East Haven, Meriden, Hartford, and New London, providing services throughout the majority of the state. We also provide in-home assisted living services for 9 Managed Residential Community facilities.
Utopia provides specialized medical home care for patients who have experienced a change in health condition and require nursing and therapy in their home. This may begin with a hospitalization or a short stay at a skilled nursing facility. Utopia functions as the final link of the continuum of care from the acute setting, to sub-acute, to post acute care. We partner with all community healthcare providers, understanding communication and collaboration are essential in care coordination and ensuring patient safety. We provide a full spectrum of home care services including Medical / Surgical, Orthopedics, and Wound Care.
Utopia is also equipped to provide long-term home care for patients who are aging in place or have chronic and stable diagnoses. Our goal is to increase independence and ensure patient safety over time, thereby reducing the chances of a hospitalization. We partner with state programs such as Medicaid (HUSKY) and the Connecticut Home Care Program for Elders (CHCPE), including their partners the Agency on Aging and CCCI. Utopia also provides nursing and aide services for clients with behavioral health needs and developmental disabilities, which can include medication management and administration.
